HART (Homeless Assistance Resource Team) operates multiple localized chapters across Sacramento County that provide direct services to unhoused individuals. 

They distribute meals, snacks, hygiene kits, clothing, blankets, tents, and bus passes, often through monthly respite centers. Certain sites also offer showers, haircuts, and access to healthcare, addiction recovery support, identification assistance, and housing referrals. 

Specific chapters, such as Arden Arcade HART, coordinate daily outreach and transportation support, while Midtown HART operates four respite centers with seating, charging stations, and on-site case navigation. 

The Youth program organizes educational outreach, job training, and housing guidance for young people.
